An Advance Directive is a legal document that allows you to communicate what type of life sustaining medical care you would want if you were in the hospital and ever became unable to speak for yourself. In other words, an Advance Care Directive is a set of instructions that express your health care desires if you are in a close to death medical.

Join us for an interactive workshop on completing an Advance Directive. All participants will be guided through this simple, yet thought provoking, process of choosing a health care agent and putting your wishes in writing related to your health care.
